Officials at University Hospital Limerick are investigating the circumstances leading to the death of 18-year-old Jessica Sheedy.
It is understood that the young woman attended the hospital for an elective surgery procedure last Friday, and died later at the ICU due to excessive blood loss.
The investigation is ongoing.
