A series of human errors led to 9-yr-old girl’s death, coroner says

A coroner has found that a young girl’s death after appendicitis surgery was caused by a “series of human errors” rather than a “systemic failure”, reports The Mirror.

Raychel Ferguson, nine, from Coshquin, Co Derry, died on June 10, 2001, at the Royal Children’s Hospital, Belfast, following surgery at Altnagelvin Hospital, Derry.

The chairman of the inquiry, Mr Justice O’Hara, found in 2018 that the deaths of Rachel and Adam Strain and Claire Roberts were the result of “negligent care”, reports RTE.

The inquiry also looked into the deaths of Lucy Crawford and Connor Mitchell.

“This has been compounded by knowing that Raychel’s death was avoidable,” Coroner Joe McCrisken said, reports The Mirror.
