New investigation launched into the death of teenager Aoife Johnston

The Health Service Executive has ordered a formal investigation into the death of a 16-year-old girl at University Hospital Limerick last year, reports RTE.

Aof Johnston, from Shannon, Co. Clare, died in hospital from meningitis in December last year.

The HSE said director-general Bernard Gloster had considered a report the agency received this month into the deaths.

“The report is significant in its findings about the care of Aoife and the operation of the hospital,” the HSE said in a statement, reports RTE.

“Having considered all of the matters, the CEO is satisfied that questions of accountability need to be addressed,” the statement added, reports RTE.

“I have appointed the former Chief Justice Mr Justice Frank Clarke (retired) to conduct a formal investigation into all of these matters, to make findings and to report to me,” Mr Gloster said, reports RTE.
