At the inquest into the death of Aoife Johnston, a medical misadventure decision was rendered, reports RTE.

Coroner John McNamara of Limerick gave his verdict at Kilmallock Courthouse following four days of testimony.

On December 19, 2022, Ms. Johnston of Shannon in Co Clare passed away from meningitis.

The 16-year-old had shown up two days earlier at the University Hospital Limerick Emergency Department with suspected sepsis, but he would have to wait a long time for treatment, reports RTE.

Mr. McNamara noted the challenging week the deceased’s family experienced throughout the inquest, which replayed the events of December 2022, and offered his deepest condolences to them.

He claimed that during Aoife Johnston’s stay in the UHL emergency department, there were institutional shortcomings, lost chances, and communication breakdowns, reports RTE.
