The man was left to languish at Letterkenny University Hospital while waiting to see an specialist at Galway.
However, his name was not added to the transfer list until 13 days later, at which point he had already expired.
A probe into the incident also revealed that there was no procedure in place to transfer patients between Letterkenny and Galway.
