The HSE has apologized to the family of a teenager who died from meningitis at University Hospital Limerick for “serious failings in the care” of the girl at the hospital, reports Breaking News.

A damning internal review of the circumstances surrounding the death of Aoife Johnston (16), from Shannon, Co. Clare, was released to her family last week and came to light on Sunday.

The report said Aoife waited for 12 hours in UHL’s very overcrowded emergency department and was not treated for sepsis until it was too late.

She was not provided with the care she needed, even though her family desperately reported that her health was deteriorating, reports Breaking News.

The review found that delays in treatment were in breach of national sepsis treatment guidelines.

She died of bacterial meningitis at UHL on December 19, 2022, two days after admission to the hospital.

“We have apologised to them for the serious failings in the care we provided to their daughter which have been identified in the review,” reports Breaking News.
