A recent probe into some 1,300 reported maternity incidents has revealed 67 “extreme” cases, which resulted in permanent disability or death of the infant.
An RTE Prime Time programme, aired on Thursday, highlighted three cases were no actual incident was reported and investigations were only initiated after threats of legal action.
Earlier this year, it was revealed that HSE acute hospitals paid out almost €67m in compensation for medical malpractice while performing birth procedures in the period between 2010-2014.
